DSRIP

Across New York, a transformation is taking place in how we provide care to people.

Union Community Health Center is proud to be playing an important role in the Bronx Partners for Health Communities (BPHC) a Delivery System Reform Incentive Payment (DSRIP) Program.

In 2014, New York State demonstrated a commitment to improving patient care and satisfaction by establishing a five-year statewide initiative called DSRIP.The goal of DSRIP is to keep people well and out of the hospital by building a provider system of coordinated, community-based care that focuses on treatment and wellness, and addresses our communities’ health disparities. 

DSRIP is redefining healthcare to focus on treating the whole person – including the social and environmental factors that contribute to poor health. Through DSRIP, we are giving doctors and other providers the ability to coordinate with each other on their patients’ care, bringing disease management and education programs to the community, exploring innovative models for care, and developing a payment system that rewards doctors for keeping patients healthy.
