For nine consecutive years the Robert Wood Johnson Foundation (RWJF) has ranked Bronx County as the “least healthy” county of all 62 counties in New York State.

Access to quality health care is a major determinant of overall health and wellness, impacting life expectancy, employment and family dynamics.

The UCHC Mobile Health Fleet brings vital health care services to thousands of Bronx residents; thereby, eliminating barriers to access.

Located in the Bronx, Union Community Health Center has emerged as one of the largest Federally Qualified Health Centers (FQHCs) in New York State.

• UNION provides health care at six major locations to over 37,000 unique patients with a service area of over 600,000 individuals, making nearly 200,000 patient care visits annually.
• UNION offers comprehensive health care services, including: adult medicine, pediatrics, rapid care, physical and occupational therapy, behavioral health, dental services and many specialty services.
• UNION’s patients and staff represent over 100 different nationalities and cultures.
• UNION employs over 250 staff and serves as an economic engine for the community.
• UNION’s holistic health care model includes primary care, behavioral health, and dental care providers designing and delivering care as one unified team for each patient.
• UNION is a teaching facility, training over 100 medical and dental providers annually who understand the complexity of serving low-income, uninsured, disadvantaged and indigent populations.

• Having achieved the best overall clinical outcomes and demonstrating high-quality across clinical operations, in 2017 HRSA recognized UNION a “Health Center Quality Leader” among the top 30% of all HRSA-nationally supported health centers.
